We are looking for a self-motivated team player to join us in the role of Community Support Lead. The aim of this role is provide practical support to adults living with low vision and blindness in the home across the county.

Location: Cam Sight, 167 Green End Road, Cambridge, CB4 1RW with travel across Cambridgeshire
Contract term: One year, potential to extend subject to funding
Hours: 28 hours per week, Monday to Friday
Salary: £21,693 pro rata
Benefits: 25 annual leave days pro rata plus bank holidays
Responsible to: Client Services Manager

Main duties

  • Undertake casework to support adults with low vision and blindness across Cambridgeshire by conducting home visits to support clients in the community. Where appropriate, make referrals to other sources of support both within and outside of Cam Sight.
  • Assess equipment and magnification needs of clients, and demonstrate equipment.
  • Advice and support on benefit applications; attend benefit hearings; completion of documentation on behalf of client e.g. PIP; liaison with associated professionals as appropriate.
  • Maintain client records and Community Support Team documentation. Log referrals and prepare specific data as required by Cambridgeshire County Council contract.
  • Promote the work of Cam Sight by establishing and maintaining effective working relationships with the local authority, health and care services.
  • Provide information and advice regarding visual impairment and relevant signposting for local authority, health and care services.
